Privacy statement and Cookie policy

Eat&Sea Therapy is responsible for the processing of personal data as shown in this privacy statement. Processing of personal data takes place in a careful manner in accordance with the requirements of the General Data Protection Regulation (GDPR) from 25 May 2018.

For proper treatment it is necessary that I, as your treating therapist, create a file. This is also a legal obligation imposed by the Medical Treatment Contracts Act (WGBO/Wkkgz). Your file contains notes about your state of health and information about the examinations and treatments carried out. The file also contains information that is necessary for your treatment and that I have requested, after your explicit permission, from another healthcare provider, for example from the general practitioner.

I do my best to ensure your privacy. This means, among other things, that I:

  • Handle your personal and medical information with care,

  • Make sure that unauthorized persons do not have access to your data.

As your treating therapist, I have sole access to the data in your file.

I have a legal duty of confidentiality (medical professional secrecy).

Definitions:
- User: every user of the website www.etandsea.nl

The data from your file can also be used for the following purposes:

  • To inform other care providers, for example when the therapy has been completed or when a referral is made to another practitioner. This only happens with your explicit (written) permission;

  • For use for observation, during the therapist's absence;

  • For the anonymized use during peer review;

  • For informing about products or services of Eat&Sea Therapy via the website or in another digital way that could possibly be of interest to the relevant User. If the User objects to this, he or she can contact Eat&Sea Therapy;

  • A small part of the data from your file is used for the financial administration, so that I or my administrator can draw up an invoice;

  • If the User requests Eat&Sea Therapy (online) for information about services, Eat&Sea Therapy processes the personal data to fulfill that request;

  • If the User books a session (online), Eat&Sea Therapy uses the personal data to establish that session;

  • If the User has registered (online) for the newsletter, Eat&Sea Therapy will use the personal data to send the newsletter to the User;

If I want to use your data for another reason, I will first inform you and explicitly ask for your permission.

You have the right to inspect your client file at any time.
You have the right to have your data removed at any time.
The data in your client file will be kept for 20 years, as required by law (WGBO).

MINORS

In some cases, Eat&Sea Therapy explicitly asks for permission to use certain personal data. If the User is younger than 16 years old, the User needs the permission of his or her parent or guardian to provide the personal data to Eat&Sea Therapy. In that case, the parent or guardian can always change or delete the data provided to Eat&Sea Therapy.

PRIVACY ON THE CARE NOTE

The care bill you receive contains the following (personal) data that are requested by the health insurer, so that you can declare this bill to your health insurer:

  • Your name, address and place of residence.

  • Your customer number and/or your date of birth.

  • Insurance and/or policy number.

  • The date of the treatment.

  • A brief description of the treatment.

  • Performance code.

  • The cost of the consultation.

From practice I work together with the following organizations with regard to the processing of your data and I have entered into a processing agreement with them:

  • Rompslomp.nl BV digital patient/client administration;

  • Google;

  • Microsoft.

Eat&Sea Therapy collects personal data from a User when the relevant User has passed on this personal data to Eat&Sea Therapy via the website, for example by contacting a request for information about the services of Eat&Sea Therapy, booking a session at Eat&Sea Therapy, or by the User has registered for the Eat&Sea Therapy newsletter.

What are Cookies?
